1) Please provide us with 2 social media handle LINKS for identity verification. Accounts must be set to public until vetting is complete and have been active for a minimum of one year. Types accepted: TikTok, X, Bluesky, Instagram, Facebook, and Substack. Your privacy is of the utmost importance to us, and these will not be shared.

2) What state do you reside in? What is your age? On a scale of 1-10, rate your proficiency with basic tech, including but not limited to: downloading apps, adjusting general phone settings, and familiarity with basic phone functions such as calling, texting, and emailing.
3) What is your biggest reason for wanting to make a difference both in your local music community and on a larger scale?
4) Have you participated in any other form of music activities? If so, to what extent?
5) Do you have children? If so, and you are comfortable, please share their ages and any concerns you have regarding leaving your child/children with someone trustworthy once the band performs.
6) Have you ever been in any leadership roles in personal or professional settings? Do you have a desire to lead or have expert knowledge in a certain area? Can you share an example of a time you successfully defused a tense situation? What is your leadership style like?
7) Please provide a minimum of 3 skills/strengths and 3 weaknesses you feel you possess. This will help us help you find a place within our club where you are confident you can make a difference. Every role is valuable, and every person's contributions matter.
8) What are you looking to receive from this club? What kind of support are you looking for? What connections do you need in order to move forward from the place you are at now? What is hindering you from doing what you need to do to help make a difference? Are money and resources holding you back? Or do you lack connections?
9) What level of commitment are you willing and able to make? What is your work-life balance like? This will help us understand who can take on more time-consuming responsibilities, and who may need smaller tasks that take less time.
